Important Notice Regarding Form 1095

In accordance with the Paperwork Burden Reduction Act, the Company is not required to automatically furnish Form 1095 to employees if a clear, conspicuous, and accessible notice is provided.

Employees who did not opt to receive a printed ACA form for the 2025 tax year will not receive Form 1095 by mail. However, a copy is available upon request.

To request a copy of your Form 1095, please contact HR Department at HR@degeschamerica.com. Requests will be fulfilled within 30 days of receipt.

This notice will remain available on the Company website for the duration required by law.
